1. NBDB Registration Form – duly accomplished
2. Articles of Incorporation and SEC Certificate of Registration
3. BIR Form 2303
4. Business Permit (current year)
5. Financial/Revenue Statement and ITR for previous year (received by BIR)
6. List of Personnel as of date of application/filing (indicate the names & designations)
7. List of Equipment (if also applying as Book Printer)
8. List of Books Published, Imported, Exported, Printed, Sold in the previous year
9. Company Profile Company Profile

Sole Proprietorship:

1. NBDB Registration Form – duly accomplished
2. DTI Certificate of Registration
3. BIR Form 2303
4. Business Permit (current year)
5. Financial/Revenue Statement and ITR for previous year (received by BIR)
6. List of Personnel as of date of application/filing (indicate the names & designations)
7. List of Equipment (if also applying as Book Printer)

Procedures

1. Accomplish the Online Registration Form on the NBDB website https://booksphilippines.gov.ph/ or through this link: www.booksphilippines.gov.ph – E-Registration NBDB Online
   ∙ Enterprises: Enterprises Beta Online Registration Form, Upload the necessary requirements on the same platform.
2. The NBDB personnel shall evaluate the submission.
3. Registrant to comply with the pre-evaluation findings – accomplish duly the online registration form and/or submit the lacking requirements.
4. If found complete and compliant, the NBDB shall submit a recommending approval to the Executive Director or her authorized representative for the approval of the Certificate of Registration.
5. Once the Certificate of Registration is signed, the NBDB personnel shall notify the registrant about its availability.
