Documentary Requirements

Bureau of Philippine Standards

  1. Duly accomplished ICC Application form – Notarized copy
  2. Packing List (Certified True Copy)
  3. Import Entry (Certified True Copy)
  4. Commercial Invoice (Certified True Copy)
  5. Bill of Lading/Airway Bill (Certified True Copy)
  6. DTI Business Name Registration – Sole Proprietor; SEC Registration for Partnership/Corporation (Certified True Copy)
  7. Summary of Batch Numbers/Serial Numbers of the products
  8. SPA/Board Resolution
  9. Surety Bond, if to be issued Conditional Release
  10. Inspection Report
  11. Test Report
  12. Warehouse contract of lease

NOTE: Documentary requirements for conditional release and certificate of exemption is same as above enumerated. Issuance of either document depends on the findings and recommendation of BPS-DTI.

PNP – Firearms and Explosive Office

For Commercial Use:

  1. Letter request addressed to the Chief, PNP (coursed through the Chief, FEO);
  2. Authenticated Copy of License to Deal;
  3. Summary of Transactions
  4. NSW Printed Application

For PNP or AFP use:

  1. Letter request of the indentor addressed to the Chief, PNP;
  2. Letter of intent by the purchasing agency addressed to the Chief, PNP;
  3. Authenticated photocopy of Indent License to Deal;
  4. Original End-User’s Certificate (duly signed by the issuing authority)
  5. Purchase Order (original/authenticated);
  6. Certificate of Availability of Funds or Approved Budget Contract (ABC), attached Special Allotment Release Order (SARO) intended for the procurement;
  7. Certificate of Training issued by the Head of Agency;
  8. Notice of Award (authenticated);
  9. Contract Agreement (authenticated);
  10. NSW Printed Application

NOTE: Requirement No. 7 is not applicable to AFP/PNP

For LGU Procurement:

  1. Letter request of the indentor addressed to the Chief, PNP;
  2. Letter of intent by purchasing LGU addressed to the Chief, PNP;
  3. Inventory of Firearms as certified by Authorized Bonded Firearm Custodian;
  4. List of Regular Plantilla Position authorized to use the firearms;
  5. Authenticated photocopy of Indent License to Deal;
  6. Purchase Order (authenticated);
  7. Board Resolution from Sanggunian Panlalawigan/ Panlungsod/ Bayan or Barangay Lupon;
  8. Deed of Absolute Donation as the case may be;
  9. Original End-User’s Certificate (EUC);
  10. Certificate of Availability of Funds or Approved Budget Contract (ABC);
  11. NSW Printed Application.

PNP – Firearms and Explosive Office

  1. Receiving Clerk checks the completeness of submitted requirements
  2. Processor process application and endorsement of documents
  3. Proof Reader checks the prepared documents
  4. Chief Clerk checks and reviews the authenticity of submitted requirements.
  5. Chief, Permits and Other Licenses Section reviews and endorses application to Chief, Firearms and Licensing Division
  6. For concurrence of Chief, Firearms and Licensing Division
  7. For concurrence of Assistant Chief, Firearms and Explosives Office
  8. For concurrence of Chief, Firearms and Explosives Office and transmittal of application to CSG MSG Center
  9. For review and concurrence of CDS, CSG and transmittal to DD, CSG
  10. For concurrence of DD, CSG
  11. For recommendation for approval of D, CSG
  12. For concurrence of TDO
  13. For concurrence TCDS
  14. For concurrence TCDO
  15. For approval or disapproval of Chief, Philippine National Police
